The debate about the rise in errors and near misses
Rx-info managing director Colin Richman has joined the debate about the rise in errors and near misses by healthcare staff giving medication to patients. The National Patient Safety Agency reported the number of cases doubling to 86,000 in two years; at least 100 patients each year were dying or suffering serious harm as a result. The agency blamed long hours, fatigue and reduced staff levels.
